Submit an Order

Once you have submitted your Request for Quotation you will receive a Quote including pricing and a prepared Service Order form.  To complete your order, follow the steps below.

 

ID Genomics will use the Customer Sample ID information you provide in the form to track your submitted samples and tailor your analysis report. A confirmation of your order will be sent to you and your samples will be processed according to your Order form as soon as the samples arrive.

Instructions for filling out the Order form

An Order form is sent to you with your price quotation.

• Review the areas in blue. (1)

• Fill out the Billing Information. (2)  Include the contact name, email and phone number for the person to contact to setup and process payment if this is not the main contact.

• Select a payment method. (3)

• If using a purchase order, supply PO#.

• If you are using a credit card, you will be contacted to provide card information.

• Provide Customer Sample Identifiers for each sample you are submitting. (4) These labels should be on each sample submitted.  See Sample Submission Guidelines for more information.

• Make sure the service, species and format information for each sample is correct. (5) Contact Us if you find a discrepancy with your Quote.

• Save your completed form and close the file.

• Print the Order form, sign in the indicated spot and include the signed order in the package with your samples. (6)

• Click here to send the Order File to ID Genomics. (7)

• A confirmation will be sent to you when your signed Service Order form is received and and payment authorization is processed.  Your signed Service Order authorizes us to perform the services indicated on your Service Order and indicates acceptance of the ID Genomics Service Agreement.

• Send in your samples to ID Genomics according to the Sample Submission guidelines after you have received your Service Order confirmation. (8)
